Online auto-entrepreneur application not accepted in my area?

I watched your video for an (art restoration) as I completed my application form online. However, it did not redirect me to chambres de metiers so had to enter the web address, it then said that an online application was not possible and to print out one of an array of forms and take to my nearest office.
Do I just need the auto-entrepreneur form? And is this basically the same as the online application? Also, on the online application it gives you a drop down choice of who will pay for the healthcare charges, how will I know what choices I have?
When it asks if I want a secondary name to be included on the healthcare, could I add my partners name? We are not married and don’t have any children. Would we need to be PACS’d?
