Modifying a declaration of business activity in France?

I want to add soutien scolaire or cours particuliers as my third business activity and I’m filling in the form on lautoentrepreneur.fr. For Section 7, the choices in the drop-down boxes don’t exactly match, but this is what I’ve chosen:
- Nouvelle activit茅 exerc茅e: SERVICES DE SOUTIEN A L’ENSEIGNEMENT
- Sa Nature: Profession lib茅rale
- Son lieu d’exercice: non pr茅cis茅
Do these look OK? For Section 9, D茅nomination du destinataire - is this just my name? I don’t have a general company name, although my main activity is English clubs for children, as a licensee authorised to use the company name.
