How does an auto-entrepreneur set up supplementary health insurance?

I have just been to get supplementary health insurance in France and they said I need a regime obligatoire from caisse primaire d’assurance maladie (CPAM). I have not been to CPAM. Do I need to go and register. I thought the attestation sante and carte vitale would just arrive reading through the notes you sent me when you . Obviously neither have.
