Filling out Form 751-SD with a PO Box setup

I received the form 715-SD and trying to fill it out. I am a little confused by Section 3 and one thing on Section 4. I am not operating my business out of my apartment and paying for the services of Les Tricolores to receive my mail and operate through them. In section 3 of this form would I just check the (En domiciliation commerciale) box and send them a copy of my agreement with Les Tricolores? Also in section 4 theres a question (Vous 锚tes inscrit 脿 la chambre des m茅tiers et d e l’artisanat.). Would I check this box as I am a Camera Operator for television shows but I dont think I am registered with them?
