Declaration for 2eme and 3eme trimestrialle on same form?

I am in the process of completing my declaration that is to be submitted by 31 October. The form states declaration 2e Trimestre 2014, however on the small print on the back it states:
Vous cr茅ez votre activit茅au 1er avril et vous opter pour un d茅claration et un versement trimestriels de vos cotisations vous effectuerez pour le 31octobre de 2 d茅clarations survivantes 2e and 3e trimestre.
Does this mean I add the two trimestres together and declare on this one form?
